OFFICE PROCEDURES

NEW PATIENTS


     We accept new patients for consultations with our cardiologists, for diagnostic evaluation and for cardiac rehabilitation.  Patients are accepted from referring physicians or directly from the patient.  Appointments will be made as per the referring physician request with patient convenience always in mind.  Please obtain and forward copies of applicable medical records from other physicians who have cared for you to the Heart Center prior to your first visit.  This background medical history is invaluable to our physician’s in preparing your plan of care.



NEW PATIENT REGISTRATION


     A new patient representative will contact you after we are contacted by your primary care physician to schedule an appointment for you.  Please arrive approximately 15 minutes early to insure we have captured all of your information.  Please bring with you your medical records, all of your current medications, your health insurance card(s) and the completed medical history form that was sent to you by the new patient representative.



APPOINTMENTS


We encourage you to make your appointments as far in advance as possible.  We recognize the importance of your time and will make every effort to meet your requests.  We ask your understanding when medical emergencies may occasionally cause us to delay or reschedule your appointment.
Please remember to bring your insurance card(s) with you on each visit to the Heart & Vascular Center.




CANCELLATIONS


     If you cannot keep a scheduled appointment, please notify us as soon as possible or at least 24 hours in advance.



TELEPHONE CALLS


     Our Heart & Vascular Center staff is ready to assist you and will direct your call to the appropriate person Monday through Friday from 8:00 a.m. until 5:00 p.m.  If your call is prompted by pain, acute discomfort or feel the situation calls for an immediate response, please give the specific details so we may handle your call appropriately.  Since our staff is actively involved in patient care, if your request is not an emergency, your call may be forwarded to voice mail and will be returned by the end of the business day.  When leaving a message, please spell your name and give the telephone number where you can be reached.  Please be prepared to provide the person who answers the call with your name and the person or department you wish to speak with.

AFTER HOURS


     There is always a Heart & Vascular Center physician on call when the office is closed to assure your needs after hours are met.  If you wish to make a call after hours, dial the number listed above and our answering service will help you reach the physician on call.  Please instruct the answering service that you wish to speak with the doctor on call and leave your name and number.  The doctor will respond as quickly as possible.  Be prepared to accurately describe your problem and list the medicines you are taking.  In case of an emergency, call 911 to dispatch an ambulance.  Once you are admitted to the hospital, the emergency room staff will notify the Heart & Vascular Center cardiologist on call.

Heart & Vascular Center observes the following holidays and the office is closed at this time.

The answering service procedures outlined above are in effect during the holiday times.




BILLING, INSURANCE AND CREDIT INFORMATION


Our business office representatives are available during office hours to assist you with any questions you may have about your bill.  We strongly encourage you to bring any special needs to our attention.  Our central business office telephone number is (941) 366-8330. We are here to help you with your billing questions.

     On every visit to the Heart & Vascular Center, please bring your insurance cards with you.  When checking-in at the Heart Center, please provide the staff with any changes that have occurred to your account.  The Heart & Vascular Center participates with Medicare and will bill Medicare for you for the services we provide.  As a courtesy, the Heart & Vascular Center will bill your insurance secondary to Medicare as well.  The Heart & Vascular Center participates with many managed care plans in the area.  Please be sure to bring any co-payment requirement and deductible with you at the time of visit so we may properly settle your account.
     Should your insurance carrier withhold payment or partial payment of your claim for any reason, we will be glad to assist you in obtaining an explanation from them.  The Heart & Vascular Center cannot guarantee payment of your claim.  The Heart & Vascular Center cannot be responsible for negotiating fees or claims with insurance companies or any other entity.  Patients are responsible for payment of medical care within a reasonable time, regardless of status of the claim.  If circumstances warrant an extended payment plan, your business office representative is available to assist you with such arrangements.  Please contact the office prior to your appointment.  You will receive a monthly statement showing all charges up to and including the current month.  These statements will cover all services provided both in our office and at the hospital by our physicians.  All patients with a current balance will continue to receive monthly statements until the account is cleared.  Please note:  claims filed to Medicare do not appear on your statement until Medicare has paid their portion of your charge(s).  Any balance remaining after Medicare has paid, will be the responsibility of the patient or supplemental insurance company, if applicable.




KEEPING YOUR ACCOUNT CURRENT


     If you receive contact or communication from your insurance company, or should you have any questions regarding your Heart & Vascular Center account while your claim is being processed, please call your business office representative immediately.  Notify your business office representative if your address, telephone number or insurance carrier changes at any time.  A final note for non-U.S. residents and patients without insurance.  Please contact the business office prior to your visit so that we can discuss payment arrangements on your account.
